- Description
- Brown, paper envelope with flap closure on either end. The envelope contains a discoloured, white booklet, commerically printed, with black and red text. The envelope is labelled across the top left corner in black text and has a stamp with black text in the bottom right corner. The top closure flap is discoloured and worn. Envelope contains cardboard folder, commercially printed on both exterior sides and right interior side in black text. Left side of folder interior has cardboard sheath attached, labelled with a white paper map. Sheath contains thin glass tubes enclosing metal needles, around 29 tubes and needles. Some tubes are labelled with white paper "flags". Right side of folder interior has yellow paper envelope attached with plastic clip and metal staple. Envelope is labelled with discoloured, commerically printed label.

Small chart listing zones and causes of hay-fever printed below. Back label of folder describes the method for using the kit and what to look for to find a positive reaction to the tests described. Left interior is labelled with a map of the United States and cooresponding zones. Tubes within the sheath are labelled with various causes of hay-fever and "glycerinated extract / for scratch testing". Right interior is labelled with "NAME OF PATIENT - DATE / ADDRESS - AGE / REACTS TO / When symptoms commence / When symptoms cease / Remarks:" "1 Complete test with control / TUBERCULIN PATCH TEST / (VOLLMER) / Lederle" is printed on yellow envelope attached to interior right side of folder, followed by a description of how to read the test. Stamped with "SAMPLE" in red and "303H896 OCT 31 1944" in blue on right side of label.
