There is a stamp on the cover from a business in Victoria, BC, however most of the stamp is illegible. The booklet was printed by Rayner, Dalheim & Co. Music Printers in Chicago.

The form has been filled out for seven items, each costing 25 cents. The form was filled out on January 10th, 1928, however never posted. The form folds up to become an envelope that seals.
